Erin is one of CCRA’s newest Care Coordinators, joining the team in April, 2021. Erin comes to CCRA with 13 years of experience working in Health & Social Services. Erin brings with her a vast knowledge of Medicaid Home & Community Based Services and Personal Care Supports. Erin started her career as a Direct Support Professional in 2008 and has served in various roles over the years, her favorite role having been a Care Coordinator for individuals on the ALI, APDD, CCMC and IDD Waivers. In Erin’s free time she enjoys being with her family, working in her garden and baking. Erin holds a special place in her heart for our elders and individuals who experience disabilities, she is very excited to reenter Care Coordination and feels blessed to continue on her journey of helping others.
